Acute pancreatitis (AP) is a common, complex, and unpredictable disease. While many cases are mild and self-limiting, nearly 20% of patients develop severe complications such as organ failure or necrosis, necessitating significant clinical attention. The American College of Gastroenterology (ACG) has developed these evidence-based guidelines to standardize management, emphasizing early supportive care and targeted intervention only when clinically indicated.
Early management centers on the prompt identification of patients at risk for severe disease. The guidelines emphasize that scoring systems are often inaccurate for early prediction; instead, clinicians should monitor for clinical risk factors like systemic inflammatory response syndrome (SIRS), obesity, and rising BUN or hematocrit levels. The primary goal is aggressive but balanced fluid resuscitation in the first 24 hours to prevent pancreatic hypoperfusion, with a preference for lactated Ringer solution over normal saline. Importantly, the guidelines move away from the traditional "nothing-by-mouth" approach, advocating for early oral feeding as soon as patients are able to tolerate it, which helps maintain gut integrity and prevents bacterial translocation.
The guidelines highlight a major shift in the management of necrotizing pancreatitis: the abandonment of urgent, invasive debridement for stable patients. In cases of infected necrosis, the current standard is a trial of antibiotics to potentially resolve the infection or allow the collection to mature for at least 4 weeks. Minimally invasive approaches, such as endoscopic or percutaneous drainage, are preferred over open surgery. Furthermore, the use of prophylactic antibiotics is discouraged, as evidence consistently shows they do not prevent infection in sterile necrosis.
For biliary pancreatitis, same-admission cholecystectomy is recommended to prevent recurrence. ERCP is strictly limited to cases with clear evidence of cholangitis or obstruction, rather than routine use. To mitigate the risk of post-ERCP pancreatitis (PEP) in high-risk patients, the guidelines recommend the use of rectal indomethacin, often in combination with prophylactic pancreatic duct stenting.
Alex: Repeated clinical assessment, especially during the first forty-eight hours. Watch circulation, organ function, and signs of a body-wide inflammatory response. Laboratory trends help, but no single test consistently predicts severity.
Sam: That leads to fluids. If patients can worsen before they look seriously ill, should everyone receive aggressive hydration?
Alex: The recommendation is moderately aggressive intravenous hydration, with extra boluses when circulating fluid volume is low. Inflammation makes blood vessels leaky, moving fluid out of circulation. Vomiting and reduced intake can compound that loss.
Sam: But replacing lost fluid and giving fluid without limit are different strategies. What comparison supports moderation?
Alex: The guideline describes a randomized trial in which moderate hydration appeared similarly effective and caused less volume overload than aggressive hydration. That supports giving fluids early, then reassessing frequently. It does not support continuing aggressive hydration regardless of the patient's condition.
Sam: And those reassessments matter particularly when the heart or kidneys already have limited capacity to handle extra fluid.
Alex: The authors explicitly urge caution in those patients. They suggest lactated Ringer solution, an intravenous salt solution, over normal saline. Both that preference and the hydration recommendation are conditional, supported by low-quality evidence.
Sam: What tells clinicians whether their fluid strategy is helping, rather than simply adding volume?
Alex: They follow heart rate, blood pressure, urine output, and laboratory trends. One measurement is blood urea nitrogen, which helps assess hydration and kidney function. A rising level is associated with worse outcomes, but it isn't a stand-alone treatment target that replaces examining the patient.
Sam: Now feeding seems like the opposite instinct. If the pancreas is inflamed, why not avoid stimulating it until recovery?
Alex: That was the traditional rationale for withholding food: put the pancreas at rest. Clinicians historically waited for pain to subside and enzyme measurements to normalize or fall. The guideline instead suggests early oral feeding in mild disease, as tolerated.
Sam: Does that mean patients must still work through clear liquids before eating something substantial?
Alex: No. The authors suggest starting with a low-fat solid diet rather than advancing stepwise from liquids. Trials support the safety of early feeding without increased major complications, and studies report shorter hospital stays. Significant nausea, vomiting, or impaired bowel movement still matter.
Sam: That's mild disease. For someone with severe disease who cannot eat enough, is feeding through the gut also preferable?
Alex: Generally, yes. Enteral nutrition means feeding through the digestive tract, including by tube. A meta-analysis involving three hundred eighty-one patients compared it with intravenous nutrition in severe pancreatitis. It found fewer infectious complications, less organ failure, and lower mortality with digestive-tract feeding.
Sam: That supports the route, but does it also establish that tube feeding should begin immediately?
Alex: No, and that's an important boundary. A later randomized trial found that immediate tube feeding did not reduce infections or mortality compared with feeding on demand. The guideline favors the digestive route, but says timing in severe disease remains controversial.
Sam: Let's move from support to procedures. Gallstones are a common cause, so does removing them urgently always help?
Alex: Many stones pass into the intestine without intervention. Without infection of the bile ducts, the guideline suggests medical treatment rather than early endoscopic retrograde cholangiopancreatography. That's an endoscopic procedure used to treat bile-duct stones, often called ERCP.
Sam: But the gallbladder can still supply another stone later. What prevents a repeat admission?
Alex: For mild gallstone pancreatitis, the guideline favors gallbladder removal before discharge. In the evidence it reviewed, recurrent biliary events brought eighteen percent of patients back after discharge without surgery, versus none after early surgery. That comparison came mostly from observational studies, with one randomized trial.
Sam: What about dead pancreatic tissue? Infection there sounds like an argument for immediate surgery.
Alex: That was the historical belief, but the guideline rejects automatic urgent surgery. In clinically stable patients with infected necrosis, antibiotics can allow intervention to be delayed, preferably around four weeks. The collection develops a wall, making drainage or tissue removal more manageable.
Sam: The qualification is stability, then, not simply whether infection is present.
Alex: Correct. Deterioration can require intervention, while selected patients improve with antibiotics alone. Preventive antibiotics are a different question: the guideline suggests against them in severe pancreatitis without an infection to treat. Better-designed trials failed to confirm the benefits suggested by earlier studies.
