Modern psychiatric diagnosis often struggles to capture the dynamic, heterogeneous nature of mental health in youth. While the clinical staging model provides a helpful developmental map—progressing from nonspecific, subthreshold symptoms to full-threshold, chronic disorders—it relies on a unidimensional view of symptom severity. This simplification often obscures the reality that individuals present with complex, transdiagnostic clusters of symptoms that change over time. This paper proposes that integrating the Hierarchical Taxonomy of Psychopathology (HiTOP)—a quantitative, dimensional system—into the clinical staging model can overcome these limitations, enabling a move toward true precision psychiatry.
By layering HiTOP’s multidimensional structure onto the stages of mental illness, clinicians can move beyond binary diagnostic labels. Instead of viewing a patient as simply being in 'Stage 1b' (risk state), the integrated model captures the specific severity of symptoms across domains like internalizing, psychosis, detachment, and mania. This multidimensional profiling allows for the quantitative tracking of how these symptoms fluctuate and evolve as a person moves through developmental stages. It also allows for the integration of non-symptom data, such as cognitive impairment and functional health, to provide a holistic view of the patient's trajectory.

This integration supports personalized intervention planning. By understanding which developmental track a patient is following, clinicians can better time their interventions. For example, if a patient’s symptoms are spreading from a seed dimension like executive dysfunction (ADHD), early targeted treatment for that core issue may prevent the later onset of secondary disorders like depression or substance use. This framework turns the 'noise' of comorbidity into actionable clinical signals.
Sam: That’s what staging adds. HiTOP supplies the symptom structure; staging supplies the framework for course and clinical decisions. Repeated measurements could show one dimension worsening, another improving, or a new problem emerging. Those changes would be interpreted alongside functioning, cognition, substance use, and physical health.
Alex: What is actually new here? Both approaches already existed, and neither sounds like it was designed to ignore complexity.
Sam: The new proposal puts a multidimensional symptom domain inside clinical staging. It also challenges the default sequence from mild and diffuse to severe and specific. HiTOP currently emphasizes adult psychopathology and does not fully incorporate developmental course. Each framework addresses a limitation of the other.

Sam: They searched PubMed for studies relevant to symptom progression and staging, emphasizing trajectories from subclinical to clinical symptoms. They supplemented that search with their subject expertise. This is a selective narrative synthesis, not a new cohort study or a systematic quantitative comparison of prediction models.
Alex: Then the developmental patterns are the heart of the proposal. How many do they distinguish, and how firmly should we treat them?
Sam: They propose five tracks, explicitly described as theoretical hypotheses, not empirically derived clusters. The first is general to specific: broad distress gradually crystallizes into a more focused clinical problem. That is closest to the sequence clinical staging already tends to assume.
Alex: Depression often appears before other disorders in the studies they cite. Does that support the broad-distress account, or could it mean something more specific?
Sam: In one transdiagnostic high-risk sample, nearly nine in ten participants who developed psychosis or mania first experienced severe depression. That supports depression’s place earlier in the sequence for those participants. But it doesn’t establish that depression was merely nonspecific distress, or that everyone follows that route.
Alex: So an earlier depressive episode could be a distinct problem, not an unfinished version of the later disorder. What is the competing interpretation?
Sam: The second track runs specific to general. Separate difficulties—impulsivity, irritability, or disregard for others’ feelings—could each expand into broader problems. The authors note that early childhood problems can already fall within the same broad symptom families as later diagnoses. Early symptoms need not be structurally vague.
Alex: Could those two accounts look identical at a single assessment? That would make repeated measurement essential rather than optional.
Sam: The authors make that point. Several elevated symptoms could represent shared broad distress or multiple specific problems. Following whether they rise and fall together, or independently, could help distinguish the accounts. The treatment implications differ: general coping support versus early targeting of particular symptoms.
Alex: What happens when the problems begin in just one area, rather than several areas at once?
Sam: That is the third track: spread from a seed dimension. Their hypothetical Evan starts with longstanding attention and executive-control difficulties. Academic failures precede low mood, and drinking later offers temporary relief from self-critical thoughts. The proposed history links later problems to an earlier core difficulty.
Alex: In that account, treating the current depression alone could miss the oldest source of impairment. How is the next track different from that spreading process?
Sam: The fourth is sequential development: qualitatively different problems appear in an ordered sequence. The review cites psychosis research describing depression, then negative symptoms, then positive symptoms. Negative symptoms include reduced motivation and emotional expression; positive symptoms include experiences such as hallucinations.
Alex: That sequence could guide monitoring without proving that one symptom caused the next. Does the framework also allow genuinely separate problems?
Sam: Its final track is parallel development: problems unfolding independently on different timelines. But the authors say empirical evidence does not generally support assuming that model of comorbidity, meaning co-occurring disorders. Their recommendation is to rule out other developmental relationships before treating problems as independent.
Alex: The proposal has plausible alternatives, but clinicians still need decision thresholds. Does combining the frameworks solve where those thresholds should go?
Sam: Not yet. HiTOP thresholds have largely relied on population norms rather than clinical need, while staging boundaries remain heuristic. The authors propose connecting quantitative profiles to distress, impairment, and support needs. Eventually, a person’s own history might help define meaningful thresholds, but that requires development and validation.
Alex: So what would make this more than a useful conceptual map, and who should read the full document now?
Sam: It needs youth-appropriate measures, developmental norms, longitudinal testing, and studies matching interventions to proposed tracks. Researchers in youth mental health, symptom trajectories, or risk prediction should start with “Five potential tracks for development of psychopathology.” Then read the implementation discussion. Everyone else should remember: a mixed symptom profile is not necessarily several unrelated illnesses, or one illness moving up a ladder.
